My name is I Made Agus Kusumadjaja, I was born in Sangsit on August 28, 1960. my mother named Ni Wayan Mindri and my father named Made Wenten Sukaryanca. I was the second of 4 siblings, an older brother and two younger sisters. I am married to Ketut Ariani and currently has been gifted by 2 children.

During school, I spent more time at Sangsit No. 1 Elementary School in Sangsit Village, Buleleng. I always try to study hard to be able to continue high school education. I studies at St. Paulus Singaraja School. After completing my junior secondary education, I continued my senior secondary education at Singaraja Preparatory Development Middle School. In high school education, there was a desire to continue my studies at the Faculty of Medicine because since I was a child accustomed to the health environment. I was able to finish high school education and finally was accepted to the Faculty of Medicine at Udayana University. At the Udayana Faculty of Medicine can be completed within 7 years.

After completing my education at the Faculty of Medicine, I served as a presidential inpres in a very remote area of ​​Kabaena Island for 3 years and a doctor who smelled bad, buton for 6 years, often participated in public health service activities. During my service many cases were found in the field of eye health so I was interested in being able to continue my education as a specialist doctor. I was accepted as a candidate for ophthalmologist at the Faculty of Medicine, Hasanudin University. Ophthalmologist education can be completed in more or less 4 years.
