Exertional heatstroke-sebuah laporan kasus

Abstrak

Exertional heatstroke is a life-threatening condition to all who engage in vigorous physical exertion. It will result in fatal multi- organ failure. The prognosis of heatstroke is directly related to the degree of hyperthermia and the duration. Thus, early diagnosis and rapid cooling are crucial for the patient’s survival. Clinical diagnosis for exertional heatstroke are patient with history of physical activity, hipertermia (core body temperature > 40°C), and central nervous system disfungtion (delirium, seizure, coma). In clinical aspek, patogenesis of heatstroke is similar to sepsis, need aggressive therapy in Intensive care, and many evidence that endotoxemia and cytokine may involeve for it patogenesis. We present a 16-year-old man who had sudden onset of hyperthermia, seizure and and uncouncious after excessive exercise. Patient did not get a good initial treatment. From first medical facility, patient was diagnosed with hemorrhagic stroke and then referred to the Referral Hospital. Exertional heatstroke was diagnosed in the emergency department (ER) of Sanglah Hospital. After prompt diagnosis and treatment at the intensive care unit (ICU), patient died after 5 days of treatment because of multi organ failure.
